Conviva Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive, value-based primary care to adults 65+. In this on-site role, you will manage acute and chronic conditions, emphasize prevention and wellness, and build long-term relationships with seniors and their families. Responsibilities include conducting exams, developing personalized care plans, ordering and interpreting tests, and collaborating with an interdisciplinary team.
